The Care Team
Medical Director
It all begins with the oversight of a physician, either your current doctor or we have a Medical Director who will step in when needed. Once an order to evaluate for hospice services has been given by a doctor, we can begin the process of determining how we can best help you and your family.
Nurse
Each patient will have a RN Case Manager that is the team liaison for patient communications. The RN Case Manager will see their patients weekly to assess symptom management, pain control, and overall patient well-being. The Nurse will seek input from family and facility care teams. He/she will offer medical guidance under the direction of the patient’s physician.
Home Health Aides
Certified Home Health Aides will see patients up to five times per week. The Aide will oversee the patient’s personal care including showers, stocking of incontinence supplies as needed, skin care, and emotional support to the patient and family. The Aides will report any change of condition or necessary patient information to the RN Case Manager who will share it with the team.
Social Worker
Our Licensed Social Workers will see the patient to assist with emotional support as well as resources and tools like financial aid, health insurance issues, living wills, advance directives and more. They will assist with arranging respite care, or help transition the patient to another living environment.
Chaplain
The Chaplain’s see patients every two weeks to offer spiritual, emotional, and grievance support to the patient and their family. Every patient’s individual belief system is honored, and the Chaplain offers reassurance and comfort.
Trained Volunteer
Volunteers may visit patients and provide companionship, run errands for exhausted family members, or be working behind the scenes in numerous ways to assist the team.
